Service & Specialties

For home health cases needing a skilled nurse, the RN will begin providing care for our patients. They assess patients' condition during their visit and chart observations. Additional tasks, depending on the patient's need, include: evaluations, including vital signs and medication review, administering medication as prescribed by the patient's physician, dressing or redressing wounds and assessing the healing progress, educating patients and their families on proper home health care strategies and procedures, coordinating with the other home health clinicians involved in the plan of care, referring palliative care to our palliative care team as needed to keep the patient comfortable, and listening and responding to any patient or family concerns.

Physical therapists help people with physical injuries or illnesses regain and/or increase their range of motion. Physical therapists, or PTs, assist those who have lost motor function due to accidents, illness or age. They also help determine the underlying cause of the trauma, counsel patients, and work with them to create a home health plan for rehabilitation. The home health plan can include stretching, strength training, exercises and physical manipulation. Sequoia Home Health physical therapists are experts at what the body can do, and will educate patients and families about what is possible, and keep patients motivated during ongoing therapy. They will also track progress and report to patients and their families.

The occupational therapist will utilize interventions to maximize safety and performance in activities of daily living (ADL's) by teaching patientss new ways of approaching tasks and educate them on how to break down activities into achievable components. They assess the home and other environments and provide advice on alternatives that may be needed to work with a patient's needs and capabilities. Additionally, they can make recommendations and train in the use of adaptive equipment, and guide and educate family members and caregivers.

The speech therapist can work with people who have trouble communicating. They can also provide swallow evaluations and provide additional assistance for those who have difficulty swallowing. Loss of function can result from stroke, multiple sclerosis, cerebral palsy, dementia, and other illnesses.

Sequoia's social workers provide patients and their families with the psychosocial support needed to cope with chronic, acute, or terminal illnesses.They provide patient education and counseling, and make referrals for other services. They are also able to provide interventions designed to promote health, prevent disease, and address barriers to healthcare access.
